Even with an occult altar, it's a pain to switch for one teleport. Lunar isle, Kharyll, and Kourend are personal favorites. If you have diaries done, grand exchange and seers village are both good choices. After that, I'd say barrows and waterbirth are good options if you engage in that content frequently.
Right click the teleport in your spell book and change it to ge. Priority in which teleport is the left-click option can be changed using the setup menu within portal nexus. I think by clicking and dragging it to the top.
Put all city teleports underneath eachother, dungeons, island etc. I exclusively use it for catherby teleport (for the catherby farming patches). All the other teleports have a better/reasonable alternative. I have mine in order of spellbook, then ranked in lowest lvl magic required to cast to highest required.
