Download the current OSHA reporting forms below. The OSHA 300 forms are the current forms required by employers to record accidents, injuries, etc. They replaced the older Forms 100 and 200. On January 1, 2002, OSHA changed the injury and illness reporting forms, eliminating the 200 form and replacing it with a new 300 series.
You must complete an Injury and Illness Incident Report (OSHA Form 301) or equivalent form for each injury or illness recorded on this form. If you’re not sure whether a case is recordable, call your local OSHA office for help.
