Unlike the OSHA log 300, which lists each incident, the OSHA 300a report tallies the numbers without detailing individual incidents. You’ll fill out this summary annually at the beginning of the next calendar year.

You can report to OSHA by calling OSHA's free and confidential number at 1-800-321- OSHA (6742); calling your closest Area Office during normal business hours; or by using the online reporting form at https://www.osha.gov/pls/ser/serform.html. Many employers are required to electronically submit information from their Form 300A Summary to OSHA.
