You can report to OSHA by calling OSHA's free and confidential number at 1-800-321- OSHA (6742); calling your closest Area Office during normal business hours; or by using the online reporting form at https://www.osha.gov/pls/ser/serform.html. Many employers are required to electronically submit information from their Form 300A Summary to OSHA.
