It took some digging, but I found a form which got them to send a password reset email that actually arrived in my inbox! It asked for my myspace url which I don't know so I just filled it in as /Idontknow and that seemed to make them happy enough.

I click 'forgot password' and it send a recovery email to my original email account associated with Myspace. Obviously that's a problem. I tried logging in with my old phone number, but there is no account associated with it.
