If you do have an IG appropriate issue, use one of the following methods to Contact the IG office. If you have the ability to complete a DA Form 1559 (Army) or AF 102 (Air) , please do so, and submit during your contact with our office.
List the names within your chain of command/supervision that you have addressed this matter with prior to contacting the IG. Describe the incident; list specific persons involved; and what standard/rule/regulation/law was violated.
When submitting a complaint, include who, what, when, where, why, and how. Complete every field on the OIG Hotline complaint form. If one of the fields is not applicable to your complaint, enter "N/A" in that field.
In order to have an IG begin work on a matter, personnel must prepare a written IG Action Request DA form 1559 or the Inspector General Personal and Fraud, Waste & Abuse Complaint Registration form 102 COMPLETELY with all supporting documents for submission to their local IG:

The OIG hotline receives, screens, and determines the disposition of complaints concerning veterans or VA that relate to potentially unlawful activity or potential violations of rules or regulations; fraud, waste, and abuse; and gross mismanagement of VA programs and operations.
