You can complete the claim form you received in your claim kit and send to MetLife via mail, fax, email or complete the claim form online. Please see Frequently Asked Questions below for more details.
If you’re completing an individual claim form, you’ll need to fill out personal details about you and the insured, such as: Insurance policy number (s) you’re making a claim on; if you don’t know the policy number, you may search for it using our policy finder at metlife.com/policyfinder/.
To be completed by the Treating Physician: Physician Statement (Form CL-40) (. *In the case of minor beneficiaries, the guardian must sign the claimant’s statement on their behalf. Each form must be notarized by a Notary Public or signed in front of the MetLife Claims Manager.
