The State of Connecticut wants to give you an opportunity to stay home instead of going to a nursing home. That is the purpose of the Home Care Program For Elders. You can find out whether you may qualify for any of the services from this program by completing this Request Form.
To apply for the Connecticut Homecare Program for the Elderly (CHCPE), or to refer an elder to this program please complete the online application through ASCEND or download the English (or Spanish) Application or you may call the Community Options Unit at the CT Department of Social Services at (800) 445-5394 or (860) 424-4904.

Apply. Eligible individuals interested in applying for CHCPE benefits can download the application form below and mail it to: Department of Social Services, Community Options, 9th Floor. 55 Farmington Avenue. Hartford, CT 06105. The application form can also be faxed to 860-424-4963.
