If your EBT card is lost, stolen, defective, or never received, you can submit a replacement request online, by phone, or in person. If you make your request online or by phone, it will take from 7 to 10 business days for you to get your replacement EBT card.
In New York City, contact the Human Resources Administration by calling (718) 557-1399. Pregnant individuals and children can apply at many clinics, hospitals, and provider offices. Call your local department of social services to find out where you can apply.
Medicaid clients who have lost their EBT cards and have a change of address, should contact the Medicaid helpline to update their contact information at 888-692-6116 to update their address. Clients can also use the MAP-751K form below to make this change and fax it to 917-639-0837.
