Where your driving licence/learner permit has been lost, stolen, damaged or defaced and is still in date (that is, valid for use), you can apply for a replacement by completing an application form D.800.
If you lose your Licence/Identity Card or if it is stolen or damaged you should complete this form, report the matter to your nearest Garda Station and have them sign and stamp this form. If you have not received your Licence/Identity Card and you have changed address since you lodged your licence application you should also complete this form.
