The mass consisted of reactive bland fibrous tissue, chronic inflammation, fibrin, myocytes, and adipocytes suspicious for lipomatous hypertrophy of the interatrial septum (LASH). Final pathology review was consistent with inflamed pericardium and LASH with a reactive benign component.
On 3D transesophageal echocardiogram (TEE) the mass appeared to be a lipomatous hypertrophy of the septum without apparent thrombus. Pathology reports of the specimen were notable for an organized clot with mild mixed inflammation, contradicting the TEE findings.
