Anatomical variabilities may cause an obstruction of the blood flow leading to venal dilatation and insufficiency.

Inferior vena cava (IVC), left ovarian vein (LOV), right ovarian vein (ROV), internal iliac veins (IIV) are presented. The valves usually occur within the main trunks of ovarian veins, however, much less frequently are present within the IIVs (only about 10% of cases).

In up to 10% of women, the right ovarian vein may also drain into the right renal vein instead of the IVC.
