Any member of The Church of Jesus Christ of Latter-day Saints may now visit any temple’s information page and then click on the prayer roll link.

You can request a name on the temple prayer rolls in two ways: Go to temples.ChurchofJesusChrist.org and find the temple you want, and then click the link “Click here to submit names.”
