ATF Form 4473, Firearms Transaction Record - also known as the ATF E-Form 4473 or the ATF Form 5300.9 - is a form used to determine whether the person purchasing a firearm from a Federal Firearm Licence (FFL) holder is allowed to own a firearm.
If more than three firearms are involved in a transaction, please provide the information required by Section A, Questions 1-5, on ATF Form 5300.9A, Firearms Transaction Record Continuation Sheet. The completed Form 5300.9A must be attached to this ATF Form 4473.
To install e-Form 4473 application click on the appropriate Download link. This will display the User Agreement page. After reading the agreement, click the appropriate Agree link to download the application. File Download – Security Warning box displays. Click Run to begin the process.
An ATF Form 4473 is also known as the Firearms Transaction Record form. Licensed firearm dealers must complete this form when they may a lawful sale. Failure to complete this form is a violation of federal law.
