INSTRUCTIONS FOR FLORIDA SUPREME COURT APPROVED FAMILY LAW FORM 12.995(a), PARENTING PLAN (02/18) When should this form be used? A Parenting Plan is required in all cases involving time-sharing with minor child(ren), even when time-sharing is not in dispute.
