We reviewed the clinical and pathologic features, prognosis, and management in the literature and describe our institutional experience to improve strategies for accurate diagnosis and treatment.

The concentration of antibodies within the specimen varies, but if the specimen is not kept warm, the antibodies will stick together and sink to the bottom of the tube, becoming undetectable. As a result, in many cases cold agglutinin disease is not diagnosed correctly for months or even years.
Cold agglutinin disease (CAD), also known as primary or idiopathic CAD, is a clonal B-cell lymphoproliferative disorder of the bone marrow that results in an autoimmune hemolytic anemia (AIHA).1-3 CAD predominantly affects elderly or middle-aged people, but it may occur in people as young as 30 years.4,5
