We provide trauma-informed and culturally responsive grief support resources for vulnerable and/or impacted youth and those who work with them so they can respond confidently to their needs. The Circle of Care is created in partnership with the Illinois Family Resource Center and Hour House.
Contained on the home page of the Illinois Hospital Report Card is an interactive counties map. You can hover over the counties to see their names. By selecting a county of the map, a list of active facilities (hospitals and ambulatory surgery centers) will be displayed.
The Map provides data on overall emergency room use as well as use associated with diabetes and asthma, preventable hospitalizations, hospital readmissions and geographic variation for these measures across the state.

The user-friendly Report Card tracks hospitals across 150 data points, including survey-based patient satisfaction, staffing and beds, readmission rates, maternal health and births, and more.
