When prescribing amox/clav, the first decision is whether the amoxicillin should be dosed at standard dose (30-50 mg/kg/day) or high dose (80-90 mg/kg/day). High dose amoxicillin should be used for any infection where Streptococcus pneumoniae coverage is needed (e.g. AOM, CAP, sinusitis).
Dosing recommendations are for usual doses to treat the most common conditions. For additional indication-specific dosing, or agents not included below, refer to the Pediatric Empiric Antimicrobial Therapy Guidelines (EATG) (idmp.ucsf.edu), or Lexi-Comp.
Oral (>4 weeks to 18 years): (Products with amoxicillin component of 400mg/5mL or 875mg per tablet). Usual dose: 25mg/kg/dose (to a maximum of 875mg) twice daily.(6) Dosing in Overweight and Obese Children: Dose based on measured body weight.
