The only difference between the two is the structure: when you use que, you need to reverse the verb and the subject, but when you use qu'est-ce que you don't need to because the verbe and subject are already reversed in qu'est-ce. Also, que is more formal than qu'est-ce que.

If the word that follows IS a verb or a reflexive pronoun, then you will use ce qui. If the word that follows IS NOT a verb or a reflexive pronoun, then you will use ce que.
Almost all subordinate conjunctions in French end in -que (either as a word ending, e.g. lorsque, quoique, or as a separate word, e.g. bien que, tant que). When they are repeated, only the que part is repeated. Lorsqu'il va venir et qu'il va voir ce qui s'est passé, il ne va pas être content.
