Patient Data. There are multiple indistinct regions of T2/FLAIR high signal intensity in the subcortical white matter, bilateral internal capsule, left thalamus and brainstem. The lesions are imperceptible in T1W images and do not show restricted diffusion in DWI/ADC images.
This case demonstrates typical imaging features of ADEM, which although characterized as a monophasic illness, individual lesions are not all at the same stage of evolution, thus some small or inapparent lesions early on can be seen to appear and mature even while other lesions regress.
