The below tables are an approximate guide only. Opiate bioavailability and response are highly variable. Wide inter-patient variation exists. Consider the need to adjust dose for conditions that increase opiate risk (eg elderly, co-morbidities, renal or hepatic impairment).
However, conversion tables can and should inform clinical judgement about switching opioids and can help clinicians to avoid gross miscalculations. We provide here two examples of opioid equi-analgesic conversion tables (A6.2 and A6.3 ) that are adapted from a leading publication on this topic ( 3 ).
The Morphine Milligram Equivalents (MME) Calculator calculates total daily morphine milligram equivalents.
Not to be used to determine dosage for converting one opioid to another or for converting between fentanyl products; for computing Morphine Milligram Equivalents (MME) in pain management.
Opioid Dose Equivalence Calculation Table In order to calculate an oral Morphine Equivalent Daily Dose (oMEDD), multiply the current daily opioid dose by the conversion factor in column 3.
