Step 1: REQUIRED FORMS - The following forms must be completed in order to enroll in the 4-H program. Step 2: PAYMENT - The following payment is required to enroll in the 4-H program. UC 4-H BASICS Fees Enrollment Fee: Each child pays an annual state program fee. This fee contributes to costs for county staff positions, online systems, statewide events and activities, and accident/sickness insurance. Some counties and/or clubs have additional program fees that support county and/or club activities.
