This is how I would use that phrase: I had an appointment with the dentist at 2:30 p.m. and I was supposed to be out by 3:00 p.m. But the surgery took us well beyond the scheduled time and I did not get out until 4:00 o'clock.

Marianne goes to extremes in either direction. EDIT: Maybe "come nella gioia era al di là di ogni riserva." To me, though "gioia" is stronger than pleasure.

Yes, "just beyond acceptable" would mean "slightly over the edge", drifting into unacceptability; in other words, it would be unacceptable by a narrow margin, as you say. It's a wonderful phrase, by the way. It reminds me of Terry Pratchett's writings, one of my favorite authors.
