If you can find an airline that fits your regular needs great, use their loyalty program. If you’re going to be bouncing around all over the place, want to go with the cheapest flights, or some combination thereof, just get a good travelers card like the Chase Sapphire or AMEX gold/platinum.
Some have programs where you register your existing credit cards and earn miles for dining out, or link to certain retailers via the airline's shopping partner site and earn for purchases. This allows them to keep activity so miles don't expire.
It's really difficult to say, every program has its ups and downs, but as LupineChemist said, you should have one for each of the three big alliances. From then on, it really depends on your destinations, your booking classes and what you're trying to do with the program.
