This form is for members seeking reimbursement for roadside assistance in the following states and territories: Florida, Georgia, northern portions of Illinois and Indiana, Iowa, Michigan, Minnesota (excluding Hennepin County), Nebraska, North Dakota, Puerto Rico, Tennessee and Wisconsin.

To submit the application, please fax the form and receipts to (714)-885-1924, submit the form and receipts to a AAA Branch Office, or mail the form and receipts to: AAA Club Services
As a AAA Member, you may request reimbursement for reasonable, unanticipated costs for your hotel, meals, and substitute transportation, up to $500 for Classic Members, up to $1,000 for Plus Members and Premier Members are reimbursed up to $1,500.

Following are average per-mile costs as determined by AAA, based on the driving costs for nine vehicle categories weighted by sales. Driving costs in each category are based on average costs for five top-selling 2022 models selected by AAA.
