Let's say there's a VFR aircraft flying along at 17,500 feet (totally legal) using a local altimeter setting of 29.42". He would be 17,500 feet above sea level.
Assuming we’re talking USA/Canada, flight levels typically start at 18000 MSL, as long as the local altimeter setting is at or above standard pressure setting (29.92). Flight levels are always flown on standard altimeter, thus they’re equivalent to maintaining a pressure altitude.
