Classification of Evaluation and Management (E/M) Services. The E/M section is divided into broad categories, such as office visits, hospital inpatient or observation care visits, and consultations. Most of the categories are further divided into two or more subcategories of E/M services.
The E/M codes (99202–99499) describe a provider’s service to a patient including evaluating the patient’s condition(s) and determining the management of care required to treat the patient.
E/M coding standards and guidelines are based on the Current Procedural Terminology codes and used to document the type and severity of patient conditions.
To understand the 2021 E/M coding changes, you need to know the basics of how E/M coding worked previously. AMA’s 2020 CPT ® code set included guidelines on using patient history, clinical examination, and medical decision making (MDM) to determine the correct level of E/M codes.

On January 1, 2023, updated code descriptors and documentation guidelines for several evaluation and management (E/M) code sets will take effect. Most notably for physiatry, this includes hospital inpatient codes, and nursing facility codes.
