Give parent’s letter to the registrar, fill-up the shifting form. Have the filled-up shifting form signed by the Dean of your previous course and the Dean of your new course. Return the signed shifting form and present your grades for evaluation to the registrar. Enrollment Proper: Get your Enrollment Slip and subjects to take from the Registrar.
