Find a passport or travel document form. Only use passport forms from this website (Canada.ca). Forms on other sites may not be official or up to date. Always download a new form each time you submit an application. We may reject your application if you don’t use the latest official form.
• Persons 16 years of age or over must use this form (PPTC 140) to apply. • Children under 16 years of age must have an individual passport (use form PPTC 142 "Child Passport Application"). If you were admitted to the USA as a permanent resident before February 15, 1977, write your Alien Registration Card
