U.S. Embassy Warsaw, Consulate General Krakow, and Consular Agency Poznan require applicants to submit barcoded passport applications (DS-11 and DS-82 forms) created using the online passport application form filler.
WHAT TO SUBMIT WITH THIS FORM: PROOF OF U.S. CITIZENSHIP: Evidence of U.S. citizenship AND a photocopy of the front (and back, if there is printed information) must be submitted with your application. The photocopy must be on 8 1⁄2 inch by 11 inch paper, black and white ink, legible, and clear.
This form is used to apply for a U.S. passport book and/or card in person at an acceptance facility, a passport agency (by a ppointment only), or a U.S. embassy, consulate, or consular agency (if abroad).

You will need to fill out a DS-11 Form if: This is your first time applying for a passport. Your current passport has been expired more than 5 years. Your passport has been Lost, Damaged or stolen. You are applying or reapplying for a minors passport (age 15 or younger)
Completed DS-11 form. This form must be completed online and printed on two pages (not back to back). The passport office will not accept hand-written DS-11 applications. If the applicant has lost a valid passport, please complete DS-64 form.
