Georgia Form G-4 is the state’s Employee’s Withholding Allowance Certificate. This form is available from the Georgia Department of Revenue (GA-DOR). Its purpose is to inform a Georgia-based employer of the amount to withhold from an employee’s paycheck for Georgia’s Individual Income Tax.
