Every Sim in the game that is capable of giving birth has its own unique Menstrual Cycle. Every cycle consists of 4 distinct segments: the Premenstrual Syndrome, the Period, the Fertile Windows (Ovulation), and the Luteal Phase. Sim’s first day of the cycle is defined by the first day they begin bleeding.

Sims that you have marked as capable of getting pregnant in create-a-sim can get pregnant and have cycles. Their actual gender identity isn't considered and people with "masculine" frames can get pregnant.
