The contact tracing form can be found here. You should submit the form within 72 hours of your return to the United States, but I believe you can do it beforehand, and the system will transmit the data to the CDC accordingly.
So I filed it out twice and I selected “same info for all passengers” . Why did I have to fill it out twice? Also, I am unable to fill out the attestation form online. It won’t load no matter what I do. Can I print out these forms and hand them in at the airport on my day of travel? Thanks a lot!
One attestation form must be filled out for each passenger age 2 years or older who is not a U.S. citizen, U.S. national, lawful permanent resident, or an immigrant (“Covered Individual”), and who is seeking to enter the United States by air travel.
