10 paź 2022 · Birth certificate copies can be obtained in the State of Delaware through the Delaware Office of Vital Statistics. Copies can acquired in a number of different ways: in person at one of three locations; via written request to the Office of Vital Statistics located in the Jesse S. Cooper Building in Dover; via telephone by calling 1-877-888-0248
Download and complete the application form to request a certified copy of a Delaware death certificate. You need to provide the name, date, place and parents of the deceased, and your relationship to them, as well as a fee and identification.
The Office of Vital Statistics holdings contain birth records newer than 72 years. If you were born after 1947 contact the Office of Vital Statistics at (302) 744-4549 or online to order a certified copy of your certificate (s).
Personal check or money order should be made payable to Office of Vital Statistics. To verify current fees, the telephone number is (302) 744-4549. Information on how to obtain certified copies is also available via the Delaware Department of Health and Social Services website.
