Contact Tracing. Please read and acknowledge the following statements: Passenger Notification and Acknowledgement for CDC Order: The U.S. Centers for Disease Control and Prevention (CDC) requires all passengers boarding an international flight to the United States to provide their contact information within 72 hours before their flight departs.
Valid nonimmigrant visa holder (excluding B-1 or B-2 visas) and citizen of a Foreign Country with Limited COVID-19 Vaccine Availability, as determined by CDC (complete F only, then sign the form to complete Attestation).
