The Belt Pouch is a craftable and lootable item added by Tool Belt that is used to upgrade a Tool Belt. Upgrading a Tool Belt increases the number of slots it has by 1 and costs some experience levels.
To use the Tool Belt hold R (default) move your mouse to the item or action you want to do. You can either click left mouse button or simply release R to switch items. It can hold 2 items without being upgraded with Belt Pouches. The Tool Belt requires level 4 Agility before it can be worn.
I've seen a few posts on here saying to not put the tool belt in the baubles section (both to get more baubles and avoid the bug where it disappears) but to instead put it in what they reference as the Tool Belt or Belt slot.
To take out the item you need to be on an empty hand, hold R again and click the item you have stored on your belt. Belts only have 2 slots but you can increase that number with belt pouches. You probably also found a pouch wherever you found the belt.
