The present study indicates that longstanding GER symptoms are linked to the presence of obvious tongues of metaplastic columnar epithelium in the distal esophagus, but the association with an irregular Z-line or CLE of at least 1 cm is not as obvious. BE is a premalignant condition in the distal esophagus [ 1 ].
Also supporting a 1-cm threshold for a diagnosis of BE is evidence suggesting that the risk of progression to high-grade dysplasia (HGD) or EAC is extremely low for individuals with a normal or irregular Z line (<1 cm).
