The completed application will be reviewed by our admission team. Whether your application is approved or disapproved, you will be notified by telephone or mail.

In need of nursing home care for a medical condition that requires services which fall within the level of care the home has resources and functional ability to provide. Complete Application for Admission (notarized page 2: Form 54). Complete CF-MED 3008 dated within 30 days of admission.
Basic admission requirements for all state veterans’ homes include an honorable discharge, Florida residency, and certification of need of assisted living or skilled nursing care as determined by a VA physician.
