19 sie 2023 · I had a email from American Airlines today. I fly to NYC on Monday. The email asked me to complete a contact tracing form, which I have done but I’m having some issue with it. Each time I input my details and submit it tells me there’s an error on the form, but doesn’t specify where. I have tried EVERYTHING and I still getting the same issue.

The contact tracing form can be found here. You should submit the form within 72 hours of your return to the United States, but I believe you can do it beforehand, and the system will transmit the data to the CDC accordingly.
My question is, I filled out the contact tracing form on the website and then today I received an email to fill it out again…? So I filed it out twice and I selected “same info for all passengers” .
14 sie 2023 · I'm flying back to the US in a couple of days and received an email from AA that I must complete contract tracing forms in advance of my flight or will be denied boarding. Tried the link in the email on both my laptop and phone.
Question. I was trying to fill out my contact tracing form online and it won't accept the US address? Im not from the states but the address is definitely valid.
