From the automated message, we learn that something breached containment, "Thresher Protocol" has been activated, and everything went to hell in a handbasket. Then the exploration logs.
Site-13 was rigged with a fail-safe device, while baseline uses nuclear devices, Site-13 was rigged with an experimental teleportation device codenamed "Thresher", that in the event of complete failure of containment would fling the site off into an alternate dimension.
